An elderly man fell at an airport. While the ambulance was still en route, a bystander with no medical training opened ChatGPT and followed its guidance: keep him still, check for head trauma and bleeding, ask about medications and what caused the fall. By the time paramedics arrived, a preliminary assessment was already underway. Consumer AI reached the patient first.

This article examines what that moment means for EMS. Consumer AI has none of the safeguards of emergency medical dispatch, no medical director, no validated protocols, no quality assurance. But it is already on scene, and crews are not trained to recognize or use what bystanders have gathered. The article argues EMS should build safe escalation pathways that keep 911 at the center, create standard ways to receive bystander observations, and integrate that data into the patient record rather than pretend it does not exist.
