Unveiling the Truth: Minnesota's EMS Report and the need for Unified National EMS Billing Reform



The "2023 Financial Evaluation of Minnesota's Ground Ambulance Industry" report (November 2023) has unearthed critical insights with national implications for the nation's Emergency Medical Services industry. While the report focuses on Minnesota, its findings resonate across the United States, reflecting a broader crisis in EMS funding and sustainability. Here's some of my initial thoughts.


The Reality of Insurance Reimbursements:

Minnesota's report reveals a concerning gap between insurance billables and actual reimbursements. With ambulance services billing $1.2 billion but receiving only $450 million, the issue of insufficient reimbursement rates becomes glaringly apparent. This discrepancy is not unique to Minnesota but is indicative of a nationwide problem in the EMS funding model, leading to financial instability in these critical services.


Dispelling the Myth of Public Funding:

One of the most striking findings from the Minnesota report is that nearly half of the state’s ambulance services receive no government funding. This challenges the widespread assumption held by the public (and elected officials) that EMS is predominantly publicly funded. Across the nation, many EMS providers face similar funding challenges, relying heavily on inconsistent and inadequate revenue streams.


Widespread Financial Struggles:

Echoing a national trend, 72% of Minnesota’s ambulance services reported financial losses, a situation exacerbated by increasing operational costs. This highlights a systemic issue where EMS providers across the country are grappling with financial sustainability, impacting their ability to deliver essential emergency services.


Call to Action:

In light of this report, the need for a cohesive and unified approach has never been more evident. To effectively address the profession's deep-rooted issues, EMS must follow in the footsteps of our nursing and firefighting colleagues by harnessing the power of a unified, national voice. This collective strategy is key to advocating for and implementing critical changes across the industry.


Central to this movement is the need to revamp antiquated insurance reimbursement systems, ensure consistent governmental support, and develop innovative funding models. The EMS profession, diverse in its operations, must come together as one to amplify its voice and influence. A united stance will make our advocacy more compelling and our message clearer.


Taking inspiration from the nursing and firefighting sectors, where a united voice has led to impactful reforms, EMS professionals must now unite in their efforts. This involves educating elected officials, conducting thorough data collection and analysis, and advocating passionately for systemic reforms. Our aim is to ensure the long-term viability and effectiveness of EMS services throughout the United States.


The well-being of our communities hinges on a strong, adequately supported EMS system. By standing together as a united front, we can drive the change needed to secure this essential public service for all Americans. The time for concerted action and unwavering commitment to the future of EMS is now. Together, we can make a lasting impact.
